Minor Model Release (parent or guardian)

The model release with a guardian block: the child's name and date of birth, the guardian's name and relationship, their signature, and — the section most templates omit — an explicit place for the restrictions the guardian sets. Restrictions are the reason a guardian signs at all, and a release that cannot record them is a release that will be ignored. It also states plainly what this document does not do: rules about images of children differ by country and by intended use, and schools, clubs, sports bodies and local authorities frequently have their own consent requirements that sit on top of, and override, a private release.

Frequently asked

Is a guardian signature enough on its own?

Not always, and that is the point of the note on the form. Where the images will be used matters as much as who signed — an organisation with its own safeguarding or consent policy will apply that policy regardless of what a private release says. Ask before the shoot, not after.

Is this legal advice?

No. This is an area where the rules genuinely differ a lot between countries and uses. Treat this as a record of what was agreed, not as clearance.
