Medication Administration Record

A record, and deliberately nothing more. It captures the medicine exactly as named on the container, the dose exactly as written on the parent's or prescriber's instruction, when it was due, when it was actually given, by whom, witnessed by whom, and the parent's prior written consent. It contains no arithmetic of any kind: it will not convert a dose, work one out by weight or age, suggest a maximum, or state an interval. If an instruction is not clear, it is checked with the parent or the prescriber — never calculated. A childcare medication record that does maths is a dosing error waiting for a busy afternoon.

Frequently asked

Why won't it calculate the dose?

Because that is prescribing, and a setting administers only what has been consented to in writing and labelled by someone qualified to decide it. A tool that offered a number here would eventually be trusted over the label.

What if the instruction on the bottle is unclear?

Then it is not given until it is clarified with the parent or the prescriber. That is the only safe answer and the form says so.
