About Me — Emergency Profile

Make the one page somebody hands over when the person it is about cannot explain themselves — in an emergency department, at an admission, on the first morning of a respite stay, or the first time a new support worker walks in. It is for an autistic person, somebody with a learning disability, somebody living with dementia, a non-speaking person, somebody whose communication changed after a stroke or a brain injury, or a child. Care settings ask families for a page like this constantly; the family almost always writes it at two in the morning on the day it is needed, on the back of whatever was nearest. This prints it properly instead. You fill in eight blocks, written in the person's own voice: who I am and what I like to be called — which is very often not the legal name and matters more than the legal name; how I communicate, how I say yes and no, how long I need to answer, and what I do when I am not understood; the three things that would go wrong first if nobody knew them; what helps me and what distresses me; my day, including when my medicine happens; how to support me, in plain instructions; and my health in our own words, with a note of where the real records are. You get a print-ready one-page SVG plus a plain-text copy you can paste into an email or a form, generated entirely on your device. Anything you leave empty prints as a captioned write-on rule, so the same run is also a blank template for a marker pen. Type is auto-fitted — it shrinks and wraps but never below the minimum sizes printed with the page, and if words still will not fit, the tool shortens them, says exactly which ones, and keeps every word in full in the plain-text copy below. Be clear about what this is. It records what the person, or the people who know them, chose to share. Nobody has checked it. It is not a medical record and it does not replace clinical records or a care plan — and it says that on the page itself, because this sheet gets photocopied and carried a long way from anything that could explain it. It holds no amounts and no numbers: the medicine line says when, and points at the actual prescription. It names no scheme, no programme, no charity and no standard.

Or open the file in a browser and print from there. One in the bag, one on the fridge, one with whoever might be asked. MINIMUM TYPE SIZES USED (points; 72 pt = 1 inch) Name I am called 28 · identity rows 11 · panel titles 11 · every body line 11 The "not a medical record" band 8.5 · grey captions 8 These are this tool's own choice, not anybody's standard — no size rule for a page like this is published anywhere we can point at, and inventing a citation would be worse than saying so. Text shrinks and wraps to fit, but never below these. Anything that still will not fit comes off the page and is named below; the text version keeps every word. HOW TO WRITE THIS — this part matters more than the layout - Write it in the person's own voice — "I", not "he" or "she" or "the client". Somebody reading it in a corridor should hear the person, not a form. - Their own words wherever you have them, even when the grammar is theirs rather than yours. "I go loud" beats a tidier sentence that is not how they say it. - Short sentences. Present tense. One idea per line. - Positive phrasing: "I can point to what I want" tells a stranger what to do; "I can't talk" only tells them what to be frightened of. Say what works before you say what does not. - Say what to DO, not only what to avoid: "tell me before you touch me", "wait ten seconds before you repeat", "talk to me first, then to whoever is with me". - Keep "what you must know about me" to three lines. Three lines everybody reads beats a page nobody reads — and in an emergency, nobody reads the page. - Write down what the person does when they are NOT understood. That behaviour is the single thing most often misread as aggression or refusal, and it is the line that most often changes how the next hour goes. - Medicine: write WHEN, not how much. Say who has the details and where the prescription is. No numbers on this page — numbers here get copied wrong. - Say where the real records live, and who to ring. This page is a doorway to the people who know, not a replacement for them. - Print it, keep one in the bag and one on the fridge, and change it whenever it stops being true. Put the date you wrote it somewhere on it. WHAT THIS IS, AND WHAT IT IS NOT - This page records what the person, or the people who know them, chose to share. It is not verified by anybody, it is not a medical record, and it does not replace clinical records or a care plan. - The tool prints your words. It contributes layout, type size and the guidance above. It knows nothing about the person, says nothing about anybody's health, and offers no view of any kind on anybody. - It holds no amounts, no numbers and no medicine names, and it never will. The medicine line says when something happens and points at the actual prescription — because a number copied by hand onto a sheet carried in a bag is a number waiting to be copied wrong. - It names no scheme, no programme, no charity and no standard. Care settings ask for a page like this under different names in different countries, and this tool speaks for none of them and is connected to none of them. - People change, and what helps changes with them. Re-read this page every few months and whenever something stops being true. NOTES 7 of 7 panels have something in them. Page Letter 8.5 x 11 in. Frequently asked

Who is this for?

Anybody who might need somebody else to explain them, and the family, friend or support worker who ends up doing the explaining. In practice that means an autistic person, somebody with a learning disability, somebody living with dementia, a non-speaking person, somebody whose speech or understanding changed after a stroke or a brain injury, and children. It is also useful for anybody who communicates fine on an ordinary day and cannot on a bad one — pain, exhaustion and fear all take words away. Care settings ask for a page like this under different names in different countries; this tool names none of them, is connected to none of them and speaks for none of them.

Is it a medical record, or a care plan?

No to both, and the page says so in a box you cannot miss, not just here where nobody will read it. It holds what the person and the people who know them chose to write down. Nobody has checked it, nothing on it is verified, and it does not replace clinical records or a care plan. Treat it as the fastest way to find out how to talk to somebody and who to ring — a doorway to the people who actually know, not a substitute for them.

Why does it refuse to hold amounts or medicine names?

Because a number copied by hand onto a sheet that lives in a bag is a number waiting to be copied wrong, and the version in the bag is always older than the version on the prescription. So the medicine line records when something happens, in your words, plus who has the details and where the prescription is. Timing genuinely matters and is worth writing down — an hour late can matter a great deal for some medicines — which is exactly why it belongs here and the amount does not. This tool supplies no name, no number and no unit, and never will.

Why is "what you must know about me" limited to three lines?

Because a page nobody reads is worse than three lines everybody reads, and in an emergency nobody reads the page. The limit is the feature. Pick the small number of things that would go wrong first if nobody knew them — the pain that will not be reported, the thing that must never be done from behind, the person who has to be in the room. Everything else has a box of its own further down, and the plain-text copy holds the lot.

Why does it ask what the person does when they are not understood?

Because that is the line that most often changes how the next hour goes, and it is the one families most often leave out. What somebody does when they cannot make themselves understood — going loud, going still, walking out, pushing a hand away, laughing — is routinely read by a stranger as aggression, as refusal, or as not caring, and the response to a misread reaction usually makes everything worse. Writing it down in advance, in plain words, turns a frightening moment into an expected one. Say what it looks like, say what it means, and say what to do.

What should I write, and in whose voice?

The person's, wherever you can: "I", not "he" or "she" or "the client". Use their own words even when the grammar is theirs rather than yours, because a stranger reading it in a corridor should hear the person. Short sentences, present tense, one idea per line. Lead with what works — "I can point to what I want" tells somebody what to do, where "I can't talk" only tells them what to be frightened of. And say what to DO: "tell me before you touch me" is worth more than any amount of description. The tool prints this guidance with every page, because the wording matters more than the layout does.

Can I print a blank one to fill in by hand?

Yes. Choose the blank template and every box prints as a captioned write-on rule at full size for a marker pen. You get the same behaviour box by box without asking: anything you leave empty prints as a rule, so a half-finished page is perfectly normal output rather than an error. Many families find it easier to fill the first one in by hand with the person, then type it up.

What happens if I write more than fits on one page?

It stays one page, and the tool tells you exactly what it could not fit rather than shrinking the type until nobody can read it. Text auto-fits down to the minimum sizes printed with the page and no further; past that, trailing lines come off the page and are listed by name in the notes. Every word you typed is still there in full in the plain-text copy underneath, so nothing is ever lost — but treat the warning as a signal to cut, because the page is meant to be read in about thirty seconds by somebody who is busy.

Does the tool judge, rate or advise anything?

No. It has no view about anybody. It offers no scores, levels, categories or verdicts, names no scale or questionnaire, says nothing about anybody's health, and makes no claim about what this page will do. It prints your words in a layout that survives a photocopier. The people who know the person are the authority on their care, and this program knows nothing about them at all.
