Patch Test Log

Product and batch or lot number, where it was applied, when, the wait you observed before the service, the result, who observed it, and the client's confirmation. The batch number is the point. A reaction is only ever traceable to a product if the batch was written down, and "the usual one" is not a record. The log holds repeat tests over time so a client's history sits on one sheet. Wait times are yours to enter from your own product instructions and local rules — this tool states none, because they differ by product and would be wrong somewhere.

Frequently asked

How long should the wait be?

This tool does not say. It differs by product and by jurisdiction and it changes; your product instructions and your local requirements govern. The form records the wait you applied.

Is a patch test needed for every service?

That depends on the service and the product. It is standard for some and not others — your training and your suppliers' instructions govern.
